How can a student receive an academic adjustment due to a medical condition?  prev next

For students with serious medical conditions, the University Health Services can provide supporting documentation for a reduced study load, an incomplete grade, or a change to Pass/Not Pass grade option. The student must be in treatment in order for UHS to evaluate the condition. Students with a physical illness or injury should start the process at the UHS Medical Records unit, room 3220. Students who are being followed for psychological care should make the request directly to their treating provider. Documentation is then taken to the student's college or professor. The UHS does not provide notes for absences, missed exams, or late assignments; students are advised to discuss these situations with their professors. However, a Confirmation of Visit indicating the date and time a student has been seen at UHS may be obtained at the Information Desk of the Tang Center in the first floor lobby.
